Online access to your records

You can securely access your health record in the NHS App or on the NHS website using your NHS login.

If you do not have an NHS login, you can create one.  You must be aged 13 or over and have an email address and phone number.  Read how to set up an NHS login.

You will need to provide a photograph of photo ID to prove who you are whilst setting up your NHS login.

If you do not have suitable photo ID, you can instead register for our online services using the form below.  We will then send you 3 registration details:

  • Linkage Key
  • ODS Code (Organisation Code or Practice ID)
  • Account ID
