Let’s Talk About It is a service provided by Hampshire and Isle of Wight Healthcare NHS Foundation Trust. They offer Sexual and Reproductive Health services across Hampshire, Isle of Wight, Portsmouth and Southampton including contraception, sexually transmitted infections (STI) testing, advice about pregnancy, HIV, sexual assault and much more.

Changing your personal details – Meon Health Practice
If you prefer, we can add a “known as” name to your record so we as a practice can call you by your preferred name.
Changing Your Title
You can change your title from Mr to Miss/Mrs/Ms or from Miss/Mrs/Ms to Mr without changing your gender.
Tell Us Your Pronouns
Pronouns replace a person’s name when referring to them. While she/her and he/him are the most common third person pronouns, some people use gender-neutral pronouns such as they/them, ze/hir/hirs and xe/xem/xyr. If you want to change your pronouns, just let us know.
Gender Change on Medical Records
Patients may request to change gender on their patient record at any time and do not need to have undergone any form of gender reassignment treatment to do so.
We must advise you, the patient, that changing your gender on your records would mean that you will be supplied with a new NHS number which is not reversible, and we will need to register you as a new patient. Your old NHS number will no longer exist and your existing medical information will be transferred into your new medical record minus any information relating to your previous identity.
To revert to your previous gender, we would need to issue you with a third NHS number and transfer your then existing medical information minus any information relating to your previous identity to your new record.

Shared Care Agreements – NHS
An NHS Shared Care Agreement is a formal arrangement between you, our practice and an NHS specialist clinic. Under this agreement, our practice will prescribe specialist medication and undertake appropriate monitoring, while the specialist clinic will continue to review your care at regular intervals. This arrangement ensures that responsibilities for prescribing are clearly defined and governed by local policies, maintaining patient safety and high standards of care.
Shared Care Agreements – Private
In recent years, more patients have sought private treatment and asked GPs to prescribe specialist medications recommended by private specialists, this differs significantly from NHS Shared Care Agreements. In line with other GP practices, we do not participate in Shared Care Agreements with private providers or issue new prescriptions for specialist medications initiated by private providers, patients must obtain these prescriptions directly from their private specialist. Shared Care Agreements with private specialists is only possible when this service is being provided for the patient on behalf of the NHS.
